The City of Maple Valley is an authorized Passport Acceptance Facility, allowing us to assist with applications for U.S. Passports.
Passport Applications are accepted at Maple Valley City Hall, located at 22017 SE Wax Road, Suite 200, on Tuesdays and Wednesdays from 9:30 a.m. - 3:30 p.m.; no appointment necessary. Please Be Aware: The actual decision to issue or deny a passport is made only by a Passport Services Agency, not the City of Maple Valley. What Do You Need To Apply For A Passport?
First-Time Application or In-Person Renewal
PLEASE NOTE: ALL APPLICANTS, INCLUDING MINORS, MUST APPEAR IN PERSON FOR A NEW OR IN-PERSON RENEWAL PASSPORT
- Application for Passport, Form DS-11: Applications are available at City Hall or click here to download an application from the State Department's website.
- Proof of Citizenship: Certified copy of birth certificate issued by State, County or City (hospital certificates are not acceptable), OR a previous passport, OR Naturalization Certificate, OR Certificate of Citizenship. Photocopies are not acceptable. Original documents must be submitted with the application and will be returned when the passport has been completed.
- A certified birth certificate has a registrar's raised, embossed, impressed or multicolored seal, registrar’s signature, and the date the certificate was filed with the registrar's office, which must be within 1 year of your birth. Please note that some short (abstract) versions of birth certificates may not be acceptable for passport purposes. Information regarding obtaining certified copies of birth certificates or other vital records is available at www.vitalchek.com.
- Current Photo Identification: Acceptable forms of I.D. are a Driver's License (not temporary or learner's permit), Government employee I.D. card, official military I.D., FAA aircraft pilot's license. (Social Security or Medicare cards, credit cards, voter registration, or club membership cards are not acceptable.)
- Two Identical Passport Photos: 2" X 2" in size, photos must be current (within the last 6 months), clear, front view with no hats or dark glasses, and printed with a white to off-white background.
- Additional Requirements for Minor:
- Minor under age 16: Both parents or legal guardian(s) must be present for applicants under the age of 16. If both parents or legal guardian(s) are not able to be present, a consent form (Form DS-3053) must be completed or other supporting documentation must be submitted. Child parent relationship must also be established for minors under 16; a previous U.S. passport is not acceptable as proof of relationship to the applying parent(s)/guardian(s). - Minor ages 16 - 17: For security reasons, parental consent may be requested. If your child does not have identification of his/her own, you need to accompany your child and present identification.
- Passport Fees/Payment Methods:
- Passport Fee: personal check or money order made payable to U.S. Department of State (no cash or credit/debit card). For multiple applications, a separate check or money order is required for each application.

What are the differences between a Passport Book and a Passport Card?
**To expedite a passport, an additional $60 per application is required.
This price does not include two-way overnight express mail delivery.

Form DS-82 can be used to apply for a passport book or a passport card by mail if you meet the following requirements:

If you answered "yes" to ALL of the questions, you may process your renewal by mail. You must be sure to sign this form before mailing. If you can't answer yes to all these questions, you will need to apply in-person as a first-time applicant.

How Long Does It Take?

- Routine Service: Processing time for new, renewal, and mail-in passport applications is approximately 6 weeks, but may take longer during peak travel periods.
- Expedite Service: If you need your passport before the normal processing time, you may request expedited service. It could take about two to three weeks to receive an expedited passport. Cost for expedited service is an additional $60.00. If your travel is critical or the approximate two-week expedited processing cuts too close to your travel plans, you will need to handle your passport on an emergency travel basis (see below).
- Emergency travel processing: If you need to travel on an emergency basis (your departure date is within two weeks or you need a visa within four weeks) you need to contact the National Passport Information Center at 1-877-487-2778 and schedule an appointment with the Seattle Passport Agency. You may access the automated scheduling system 24 hours a day, 7 days a week to schedule an appointment. When you appear for your appointment, you must have proof of immediate travel (airline ticket and/or original itinerary). The Seattle Passport Agency is located in the Henry M. Jackson Federal Building, 915 Second Avenue, Suite 992, Seattle, Washington. They are open 8:00 a.m. until 3:00 p.m., Monday through Friday by appointment only.
